Photocopy of registration papers on each horse entered

Photocopy of USEF membership cards for owner, trainer, rider,
driver, handler (incl. amateur certification)

Photocopy of AHA membership cards for owner, trainer, rider,
driver, handler

Sale or lease contract (if applicable)

Your check for the full amount

Make sure signatures are included for owner, trainer,
riders/handlers, and parents for minors.
